How to Set Up Fall Detection on Apple Watch

📋 Which Apple Watch Models Support Fall Detection?

  • Apple Watch Series 4 and later (Series 4, 5, 6, 7, 8, 9, 10)
  • Apple Watch SE (1st and 2nd generation)
  • Apple Watch Ultra (1st and 2nd generation)
  • Requires watchOS 5.1 or later (latest watchOS recommended for best accuracy)
  • The paired iPhone must be running iOS 12 or later

Note: Fall Detection is automatically enabled for users aged 55 and older (based on the age entered during Apple Watch setup). For users under 55, it must be turned on manually.

🔧 Step 1: Enable Fall Detection

From Your iPhone:

  1. Open the Watch app on your iPhone
  2. Tap the My Watch tab at the bottom
  3. Scroll down and tap Emergency SOS
  4. Toggle on Fall Detection
  5. Choose when it is active: Always On (recommended for elderly users) or Only During Workouts

Directly on Apple Watch:

  1. Open Settings on your Apple Watch
  2. Tap SOS
  3. Tap Fall Detection
  4. Toggle it on and select Always On or Only During Workouts

Recommendation: For elderly users or anyone at risk of falls, always select "Always On." The "Only During Workouts" option only activates detection when a workout session is running.

🚨 Step 2: Set Up Emergency SOS Contacts

Emergency contacts are the people who get notified with your location when Fall Detection triggers an emergency call.

  1. Open the Health app on your iPhone
  2. Tap your profile picture in the top right, then tap Medical ID
  3. Tap Edit
  4. Scroll down to Emergency Contacts and tap Add Emergency Contact
  5. Choose a contact from your address book and assign their relationship (spouse, parent, child, etc.)
  6. Add multiple contacts — at least 2 is recommended
  7. Tap Done to save

These contacts will receive a text message with your current location when an emergency call is triggered. The message updates with your location if you move.

🏥 Step 3: Fill Out Your Medical ID

When emergency responders arrive, they can view your Medical ID from the lock screen of your iPhone or Apple Watch. This information can be life-saving.

  1. In the Health app, tap your profile > Medical ID > Edit
  2. Fill in as much as possible:
    • Medical conditions (diabetes, heart disease, epilepsy, etc.)
    • Medications and dosages
    • Allergies (especially drug allergies)
    • Blood type
    • Weight and height
    • Organ donor status
  3. Toggle on Show When Locked — this allows first responders to see your Medical ID without unlocking your devices
  4. Toggle on Share During Emergency Call to automatically send your Medical ID to emergency services when supported
  5. Tap Done

📖 How Fall Detection Works

Understanding what happens during a detected fall helps users stay calm and respond appropriately.

  1. Detection: The Apple Watch detects a hard fall using its accelerometer and gyroscope. It measures sudden impact followed by a distinctive wrist motion pattern
  2. Alert: The watch taps your wrist and displays an alert asking if you are okay. An alarm also sounds
  3. User response: You can tap "I'm OK" to dismiss, or tap "Emergency SOS" to call immediately
  4. No response (about 1 minute): If you do not move or respond, the watch begins a 30-second countdown with haptic taps and an audible alarm that gets louder
  5. Automatic call: After the countdown, the watch automatically calls emergency services (911 in the US) and plays an audio message stating that a hard fall was detected, along with your GPS coordinates
  6. Contact notification: A text with your location is sent to your emergency contacts
  7. Location sharing: Your location continues to update and is shared for approximately 24 hours

⚠️ Important Tips and Considerations

  • False alarms: Certain activities (roller coasters, intense workouts, clapping hands forcefully) can trigger false detections. If this happens frequently, keep the watch in Workout mode during those activities
  • Wear it correctly: Fall Detection is most accurate when the Apple Watch is worn snugly on the wrist. A loose watch may not detect falls reliably
  • Cellular vs GPS only: A GPS-only Apple Watch requires the paired iPhone to be nearby for the emergency call. A Cellular model can call 911 independently — highly recommended for elderly users who may not carry their phone everywhere
  • International emergency numbers: Apple Watch automatically calls the local emergency number based on your location (112 in Europe, 999 in the UK, etc.)
  • Battery life: Keep the Apple Watch charged. A dead watch cannot detect falls. Consider charging it briefly each morning during a routine
  • Wrist detection: Make sure Wrist Detection is enabled (Settings > Passcode > Wrist Detection on the Watch). Fall Detection depends on this feature
